THIS IS A LEGAL DOCUMENT AFFECTING YOUR RIGHTS AND RESPONSIBILITIES
PLEASE READ IT CAREFULLY BEFORE SUBMITTING

#1) I hereby grant to Brandon G Notch ("Producer") the right to take still pictures of me and my tattoo/artwork or shop. I agree to release my rights to the Producer, of the photographs or other reproductions of my work that are taken or submitted to be used in print media. ("Los Angeles Tattooing") book.
#2) The Producer have no obligation to me whatsoever. Without in any way limiting the foregoing, I acknowledge and agree that the Producer are under no obligation to use the still pictures and materials (content).
#3) I represent and warrant the following (a) I am not, and during six months from today do not intend to be, candidate for any public office.
#4) I understand I will NOT be paid any compensation for appearing in the book, in the advertisements, and in the material.
#5) I know the Producer will incur significant cost and expenses in reliance upon this agreement, so I will not attempt to cancel it or to revoke any of the rights granted to the Producer herein.
#6) I acknowledge that I am a volunteer and that I shall not be deemed to be an employee of the Producer, nor shall I be entitled to the benefits provided by the Producer to its employees.
#7) To the maximum extent permitted by law, I agree that I will never sue the Producer (Brandon G Notch), its licensees or assigns or anyone because the Producer did not take or use the photos and materials or because I do not like the manner in which the Producer took or used the photos and materials. I, for myself and on behalf of my heirs, executors, agents, successors or assigns, hereby release, hold harmless, promise not to sue and forever discharge the Producer, subsidiary and affiliated companies, or publisher as well as sacred saint studios, inc. and platforms on which the book is publication. Or publicized otherwise exhibited or distributed and the sponsors thereof (the Òreleased partiesÓ) from any and all claims, actions, damages, losses, liabilities, costs, expenses, injuries or causes of action whatsoever that in any way are caused by, arise out of or result from this agreement, my appearance in the book. The book or the advertisements, on any legal theory whatsoever (including, but not limited to, personal injury, rights of privacy and publicity, defamation, or false light), regardless of whether caused by the negligence or willful misconduct of the released parties or anyone else connected with the book or the photos and materials. I will defend, indemnify and hold the released parties harmless from any and all such claims, actions, damages, losses, liabilities, costs, expenses, injuries or causes of action, as well as all those that in any way are caused by, arise out of or result from any breach or alleged breach by me or any of the representations, warranties or covenants made by me in this agreement.

I hereby agree to release to (Brandon G Notch) the Producer of the book, my rights to photographs and materials, or other reproductions of tattoo and artwork to be used in articles, advertisement, and print media for the book.

Thank you for agreeing to submit your information. Tattoo artists submitting still pictures and information for this project will be published in a hard cover book. This is a great opportunity to be part of an amazing book. Also get exposure among tattoo artist and tattoo fans around the world. I understand that by submitting my answers, photos and content, I am allowing full reprint rights of the information. I also understand I am not being paid for this information nor will I be paid in the future. The author ("Producer") or publisher may use the content as needed in the publication as long as my contact info and business name are intact and clearly printed with my answers and photos.
